What You Should Report
PULIC Standard and Dental Policies
It is essential that you notify the PULIC Claims Office if an incident occurs that may lead to a claim or if a claim is made against you. PULIC policies require that claims be reported within 10 days of receipt or before the cancellation or nonrenewal date of the policy, whichever occurs first. You should report:
- A demand for money
- A verbal or written threat of legal action
- A legal document naming you as a defendant or witness
- An attorney’s notice or letter of intent
- A request for arbitration
- A request for deposition or interview
- A subpoena, summons, complaint, notice of a lawsuit, or small claims court notice addressed to you
- Any incident reasonably likely to involve this insurance
PULIC S800 Policies (Restricted Policy Form)
The S800 policy requires written notice from the injured party or the injured party’s representative in order for a claim to be reported. A claim must be reported by you and received by PULIC within 10 days of your receipt of written notice from the injured party or before the expiration of the Claims Reporting Period, whichever occurs first. You should report:
- Written notice seeking or demanding money
- Written notice of legal action
- An attorney’s notice or letter of intent
- A subpoena, summons, complaint, notice of a lawsuit, or small claims court notice addressed to you
How to Report a Claim
Please note that PULIC does not accept reports via e-mail.
When reporting a claim, please provide the following information:
- Patient’s name, gender, date of birth, Social Security number, and employment status
- The names of any other physicians or medical providers, including hospitals and clinics, involved in the patient’s care
- The chronology of the medical treatment provided
- The nature of the claim
Report your claim to the PULIC Claims Office by certified or overnight mail, fax, or telephone.

Additional Steps
- Do not add to, delete from, or otherwise alter the medical record.
- Place the medical record in a safe place and keep a log of who requests copies and when those copies are requested.
- After making a report to us, keep copies of claim-related correspondence in a safe place—not in the patient’s chart—for future consultation. Make a note of the number we assign to the claim.
- Always require the signature of the patient or the patient’s legal representative when releasing a copy of the medical record or any information within it.
- Do not discuss the event with anyone other than your claims specialist or defense attorney.
